Responsibilities

* Responsibility for People Management: provide advice, training and support to less experienced speech and language therapists, assistants and volunteers; contribute to CPD sessions, supervision and other types of formal and informal training; participate in Practice Supervision sessions; promote the SLT role within the wider MDT and Livewell framework; recognise breakdown/conflict and seek resolution with support from Team Leader SLT; reflect on practice individually and with peers/mentors to identify strengths and development needs to raise clinical competence.
* Responsibility for financial and/or physical resources: notify senior staff of equipment updates to meet caseload demands; maintain equipment records and ensure safe use and security of clinical equipment; provide clinical activity data and routine recording of CPD, travel and related activity in line with data protection guidelines; document day-to-day financial matters per departmental policy; ensure clinical equipment is used in accordance with Livewell safety and GDPR policies; identify need for high-tech electronic communication systems to meet patient needs.
* With support from more experienced colleagues, provide onward referral to AAC West or identify appropriate aids and potential funding sources; train patients, carers and MDT members in the effective use of communication equipment.
* Responsibility for administration: ensure clinical records comply with professional registration and Livewell Southwest policies; ensure adaptations to communication and media comply with Accessible Information Standards; support general day-to-day admin and department running, including handling telephone calls and clinical/non-clinical administration as required.
* Responsibility for people who use our services: independently manage personal caseload with reference to theory, published research and evidence; collect, analyse and interpret case-related information from patients, carers and relevant sources; undertake a range of formal and informal assessments as indicated; reach differential diagnoses; assess, manage and advise patients with dysphagia and their families with support from more experienced SLTs; attend MDT and patient meetings to update goals and care plans; develop patient-centred treatment/management plans with patient/carer involvement.
* Write and disseminate clinical reports based on assessment and treatment findings to appropriate professionals; manage triage, discharge and caseload without direct supervision; communicate complex information to patients with varying levels of understanding; explain complex and sensitive information to patients and carers; demonstrate empathy when delivering unwelcome information; monitor changes in patient condition and adapt plans accordingly; identify and communicate risk factors for impaired swallow and communication difficulties; recognise when onward referral for specialist SLT intervention or multidisciplinary advice is needed.
* Negotiate with carers/patients and professionals around case diagnosis and management where there are differences in professional judgement; train and advise patients, carers, professionals and non-professionals on supporting speech, language, communication and dysphagia; contribute to multidisciplinary and uni-disciplinary teams to ensure coordinated care.
* Implementation of policy and service developments: maintain HCPC and RCSLT registration; be professionally accountable for independent clinical judgments; progress and develop professional skills through CPD and maintain a personal portfolio; recognise professional boundaries and seek advice as needed; comply with organisational policies, governance, health and safety, risk management, mandatory training and data protection requirements; follow local and national policies with professional judgement and participate in clinical governance, research and audit as appropriate.
* Other responsibilities: monitor satisfaction of people using the service and support change in collaboration with the MDT; with support from senior staff, manage emotionally complex scenarios involving patients and families. The post includes a flexible 37.5 hour week, including bank holidays.


Communications and Relationships

* Communicate sensitive and complex information with a range of staff internally and externally; adapt communication styles to support patients with significant communication impairments.


Physical Demands of the Job

* Working in awkward spaces or positions (including during home visits); supporting physically unwell patients; exposure to unpleasant smells and bodily fluids; work with distressed or anxious/aggressive individuals and families; discuss difficult topics related to communication and swallowing; use manipulation skills for therapy tools and interventions; work with people with severe physical disability, cognitive impairment and communication problems; maintain concentration under interruptions and during unpredictable clinical work; operate a VDU daily.
