Overview
Position: Macmillan Urology Specialist Nursing – Cancer Care Coordinator within the Prostate Easy Access Support Service (PEASS). This role delivers a personalised stratified follow‑up programme for patients who have completed treatment for prostate cancer.
Work location: The Christie, Manchester. The role supports the Uro‑Oncology Clinical Nurse Specialist team in delivering Personalised Cancer Care (PCC) and care coordination for urological cancers.
Key Responsibilities
* Assist the Urology CNS team in delivering PCC components: Holistic Needs Assessment (HNA), Care Planning, Health & Wellbeing offer, Treatment Summaries and Personalised Stratified Follow‑UP (PSFU).
* Provide patient advocacy, navigation through health and social care, and emotional and practical support.
* Coordinate referrals to primary, community and specialist services; maintain accurate records of onward referrals.
* Triage telephone and email enquiries, initiate appropriate responses under risk assessment protocols and, when necessary, escalated to the senior nursing team.
* Conduct pre‑planned telephone calls to assess ongoing needs and enable preventative care.
* Coordinate handover of patients to other teams for seamless transition of care.
* Support the delivery of Health & Wellbeing events, support groups and signpost to relevant services.
* Document all patient care activities accurately in relevant systems.
* Coordinate and organise appointments, including fast‑tracking or rapid re‑entry as required.
* Respond appropriately to clinical emergencies, following organisational policies and procedures.
* Provide general cancer information and support to patients, encouraging self‑management.

Qualifications & Experience
* Enthusiastic, motivated and proactive individual.
* Experience and knowledge of treatment pathways and specialist care for urology cancer patients preferred.
* Strong interpersonal and administrative skills, confidence in liaising across multi‑professional teams.
