This is an opportunity to be part of a Brand New Team ! About the Role CJP Outreach Services is looking for caring, motivated, and enthusiastic individuals to join a brand-new team supporting a young man taking his first steps into adult life. D is a young man who will be moving out of his children's residential placement into his new home in Bristol and will be supported on a 3:1 ratio. D is very active, and really enjoys exercise like lifting weights, riding bikes or going for hikes with his staff. He loves outdoor swimming, and anything physical and outdoorsy. D will need a team around him that can keep up with his active lifestyle. D is a keen collector and collects perfume bottles as well as cameras and typewriters. D is very practical, and enjoys taking things apart and rebuilding them, including bikes. He has been working on taking a fruit machine apart, which he is going to turn into a chair for his sensory room! If you have practical skills you can bring to work, this will be key to building relationships with D. D is also building his cooking skills, and has an adventurous diet and loves being supported to try new foods from different cultures. You will work as part of a dynamic, resilient team that values diversity, promotes inclusion, and encourages a positive and supportive working environment. About You We are looking for people with experience in supporting individuals with learning disabilities, autism, and behaviours of concern, ideally within community-based settings such as supported living or residential care. Your background can be children's or adult's services. Knowledge of Positive Behaviour Support (PBS) is a real advantage as well as knowledge of the impact of developmental trauma and experience working in a trauma-informed way. There may be times where physical intervention or breakaways are required to keep D and others safe. In-house person-centred training and guidance will be provided. Most importantly we are looking for upbeat, motivated and resilient staff to help D take a big next step. Staff will be involved throughout the transition process and be part of a brand-new team. Team working will be essential, as well as clear direct communication with D and your colleagues. D may find the change difficult and will need staff that can be resilient in the face of challenges and approach support with kindness and compassion. Relationships building takes time, but relationships are the key to helping D live a successful life. Staff with a full clean driving license are preferred. Full DBS check required as part of the role. Key Responsibilities * Deliver person-centred support, focusing on individual strengths and goals. * Work alongside the people we support to develop and follow personalised support plans. * Help with everyday skills such as cooking, personal care, budgeting, and household tasks. * Support health and wellbeing, including medication, medical appointments, and healthy lifestyle choices. * Encourage social inclusion by supporting access to community activities and building meaningful relationships. * Use PBS strategies to reduce behaviours of concern and promote positive change. * Keep accurate records of progress, outcomes, and daily activities. * Collaborate with colleagues, families, and professionals to ensure consistent, holistic support. Hours Rate of pay : £16.50 PH £80 sleep in rate * Support is provided on a 24/7 basis with a staffing ratio of 3:1 in the daytime, 2:1 overnight. * You will work on a rotational Rota basis covering 12-hour shifts, including every other weekend. The role will also include sleep-in duties. The individual currently lives in Bridgwater (Somerset) but is due to move to his Bristol accommodation shortly. You may be asked to support with transition visits. There is an expectation that the established support team will move with him.
