Senior Community Support Worker

We are seeking to recruit a motivated, enthusiastic individual into this challenging but rewarding post. The senior support worker will work closely with the community learning disability nurses, supporting them with all aspects of care within the Service. With a particular focus on assisting with assessment, planning, implementing and evaluating care packages as designated by the multi‑disciplinary team.
The post is based at 2 Jubilee Close, Tickhill Road Hospital.
Main duties of the job
You will be actively involved in the implementation of the agreed care/intervention programmes, working under the supervision of the community nurses. You will support individuals in a variety of settings as well as within their own homes.

Person Specification
Qualifications
* English and Maths at level 2 or equivalent (GCSE 9‑4 or A*‑C or functional skills level 2)
* NVQ Level 2 in a related subject
* NVQ level 3 in related subject or willingness to undertake a relevant Level 3 Apprenticeship
Experience
* Demonstrate a keen interest caring for others
* Previous experience of caring for patients in a clinical setting
* Previous experience of working as part of a team
* Previous experience of supervision of staff
Knowledge
* Understand importance of maintaining confidentiality
Skills
* Good interpersonal skills
* Ability to communicate effectively both face to face and in writing
* Able to support and reassure patients/carers in distress
* Basic IT skills

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary: £26,299.62 to £27,476 a year, pro rata.
