Responsibilities
To provide excellent, high-quality person-centred clinical care that advocates for patients, strive to provide a service fit for today and the future, develop personal leadership to support clinical growth. The post is a key part of the OT discharge service supporting registered staff with the assessment and rehabilitation. This role includes:
* Assessment of trauma and elective patients who have been referred to Occupational Therapy
* Taking equipment out to patients’ homes and fitting items (training will be given)
* Setting up / providing follow up visits for prescribed equipment
* Maintenance of stock
* Liaison with outside agencies and patients’ families
* Teaching patients and their families how to use and fit the equipment
* Undertaking falls assessments
Location and travel
The post is based at Frimley Park Hospital but applicants must be able to travel between all Frimley Health's hospitals and patients' homes.
