Overview
The Confederation, Hillingdon CIC is recruiting a Clinical Pharmacist to join our Synergy PCN in Uxbridge on a 12 month fixed term contract for maternity cover. The successful candidate will work across multiple practices, developing this role closely with GPs and as part of a multidisciplinary team under the guidance of a delegated clinical supervisor.

Responsibilities
* Work as part of a multidisciplinary team to develop and run processes for repeat prescription authorisation, management of medicines on transfer of care and systems for safer prescribing. See and treat patients, manage long-term conditions and promote self-care.
* Primary responsibilities include patient-facing clinical medication reviews, long-term condition clinics, care home and domiciliary reviews, management of common/minor/self-limiting ailments, medicines support, and providing medicine information to staff and patients.
* Lead or contribute to medicines optimisation, safety, quality improvement and education within the practice and PCN.
* Signpost to community pharmacy and other healthcare professionals as appropriate; refer to GPs or other professionals when needed.
* Assist with repeat prescribing policy, monitoring tests, risk stratification, service development and CQC-related activities.
* Contribute to the development of new services and treatment pathways and participate in audits of prescribing with action plans in collaboration with the practice team.
* Collaborate with GP practices, CCG colleagues, and other services within the PCN and neighbouring networks to ensure consistency of patient care.
* Support continuity of medicines supply, reconciliation after hospital discharge, and management of medicines in care homes and after discharge.
* Demonstrate communication effectively with patients and colleagues to gain cooperation and integrate practice with community and hospital pharmacy teams.
Knowledge, Skills and Experience
* Undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council.
* Minimum of 2 years’ experience as a pharmacist.
* Experience and awareness of common acute and long-term conditions likely to be seen in general practice.
* Working towards or holding an independent prescribing qualification (not mandatory).
* Ability to follow legal, ethical, professional and organisational policies and codes of conduct.
* Involves patients in decisions about prescribed medicines and supports adherence in line with NICE guidelines.
* Good prioritisation, problem-solving, and the ability to refer to seniors or GPs when appropriate.
Education, Training and Development
* Demonstrates ongoing personal and professional development; engages in regular supervision and reflective practice.
* Adheres to organisational policies including confidentiality, safeguarding, lone working, information governance and health and safety.
* Willing to enrol on CPPE pathway training programme for primary care and participate in formal education delivery as required.
Leadership and Collaboration
* Understand and contribute to pharmacy governance and workplace vision; strive to improve quality within service constraints.
* Motivate self to achieve goals; promote diversity and equality in people management.
* Collaborate within the Primary Care Network Multidisciplinary Team and liaise with CCG colleagues and other services to ensure consistent patient care.
* Build and sustain relationships with stakeholders including patients, GP staff, social prescribers, physiotherapists, physician associates, paramedics, community pharmacists, hospital staff, and other allied health professionals.
Special Working Conditions
* Travel between practice sites (where applicable) and attend meetings hosted by other agencies as required.
