Overview
We are looking for a Community Wound Care Nurse to join our Community Nursing team. This role involves delivering high quality advice and assessment for patients with complex or long term wounds, driving quality improvement to enhance patient outcomes and leading initiatives focusing on lower limb care. The post involves use of electronic clinical records through EMIS; a work iPad and iPhone will be provided. This role is suitable for experienced community registered nurses.
Job responsibilities
Provide high quality, specialist care to housebound patients with complex wounds. Advise and support the Community Nursing Teams as required, acting as a source of expertise in managing complex wounds. Demonstrate sound clinical judgement, role model to junior staff and students, assess complex patients and plan their care needs. Embedded in the Guildford and Waverley Community Nursing team, assist in delivering routine planned care such as medication management while focusing on advice and assessment for patients with complex or long term wounds, including lower limb wounds. Ensure that patients with complex wounds have a comprehensive holistic review and that care plans reflect evidence based practice.
Person Specification
Qualifications
* Registered Level 1 Nurse with current Nursing and Midwifery Council registration
* Post Graduate Tissue Viability Qualification or willingness to undertake within 18 months of being in post if post continues
* Evidence of BSc level / level 6 study
* Assessor / NMC Supervisor / Mentorship
* Evidence of recent and relevant continuing professional development or experience in wound care
Knowledge and Experience
* Holistic lower limb wound assessment and management qualification - Handheld / automated Doppler
* Assessment and Compression bandaging competencies
* Advanced range of Community clinical skills and ability to undertake full assessments and physical assessments of patients including those with multiple pathology and complex health and social needs in the Community

* A full UK driving licence and vehicle for business use if required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010
* Disclosure and Barring Service Check as part of the Rehabilitation of Offenders Act (Exceptions Order) 1975
