The Assessment Nurse is responsible for conducting comprehensive health and care assessments for individuals requiring clinical evaluation. This role involves identifying care needs, contributing to care planning, and ensuring timely and appropriate referrals. The nurse does not prescribe medications but collaborates with prescribing professionals and the wider healthcare team to ensure holistic, person-centred care.
Key Responsibilities:
1. Conduct initial and ongoing assessments of patients in a variety of settings (e.g., home visits, clinics, care homes, or hospital discharge).
2. Evaluate physical, emotional, mental health, and social care needs.
3. Complete risk assessments and safeguarding referrals when required.
4. Develop person-centred care plans in collaboration with patients, families, and multidisciplinary teams.
