Community Healthcare Support Worker

We are a busy happy team who welcome new staff members and encourage staff development and learning. We are seeking Community Healthcare Support Workers to join us in providing care and treatment to adults who have long term or complex health needs.

We are recruiting to two roles – one in Fareham (22.5 hours per week) and the other in Gosport (30 hours per week). Please specify on your application which role you are interested in.

The scope of your role will focus on supporting admission avoidance, promotion of independence and assisting in the effective management of patients with long term or complex conditions. You will be working in a multi‑disciplinary way alongside a range of primary care services including those from GP's, social and voluntary sector. You will be responsible for managing your allocated work on a day‑to‑day basis ensuring patients are prioritised according to clinical need and that assessments and care plans are recorded in line with the Trust's standards.

To support you within your role, the Trust have an excellent learning and development department, to further develop your clinical skills.

Our service runs 7 days a week and therefore this position requires flexible working and all requests will be considered.

Access to a vehicle and possession of a full driving licence is essential.


Main duties of the job

To work within the multi‑disciplinary Integrated Care Team that delivers community‑based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners.

The job will include home visiting and lone working without direct supervision.

The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and older people with mental health problems.

Qualifications

* Literate and Numerate NVQ 3 in health care related subject or willingness to undertake Full driving licence and access to a vehicle for work purposes
