About the Job:
Auckland Surgery seeks a dedicated Health Care Assistant to join our primary care team. This role focuses on supporting preventive healthcare services, while providing essential clinical and administrative support to ensure high-quality patient care within our practice.
Main duties of the job
* Take detailed patient medical histories and family health backgrounds
* Perform venipuncture and collect blood samples for diagnostic testing
* Carry out vital sign monitoring including blood pressure, pulse, and height measurements
* Support vaccination programs including seasonal flu immunizations
* Maintain accurate clinical records and input data into practice management systems
* Perform simple dressing changes
* Support the surgery in achieving QOF points, help with long- term condition reviews eg. Diabetic physical checks
* Administer B12 injections

Clinical Responsibilities
* Take detailed patient medical histories and family health backgrounds

* Evaluate cardiovascular, diabetes, and kidney disease risk factors using established protocols
* Follow current NHS guidelines for health screening procedures
* Coordinate follow-up care based on assessment outcomes
* Collaborate with clinical staff regarding patient results requiring medical review
Patient Care Management
* Identify and contact patients eligible for health screening programs
* Schedule appointments and manage clinic capacity effectively
* Handle patient inquiries with professionalism and confidentiality
* Reduce non-attendance rates through effective patient communication
* Collect pre-appointment information to optimize consultation time
Person Specification
Qualifications
Essential
* NVQ Level 3 in Health and Social Care/Care Certificate or equivalent
* Previous experience working as a HCA or HCSW
* Evidence of appropriate continuing professional development and education to maintain up to date knowledge
Desirable
* Foundation degree or Associate Practitioner qualification
* Leadership/supervisory training
* Training in coaching, motivational interviewing, or care navigation
Experience
Essential
* Minimum of 2 years experience working as a Health Care Assistant
* Experience in a primary care, GP surgery, or community health setting
* Experience working in a multidisciplinary team (MDT)
* Experience conducting clinical observations and holistic assessments
* Understanding of protocols and clinical guidelines
* Ability to manage change
* Ability to support patients to change lifestyle
* Good understanding of clinical governance
Desirable
* Motivational interviewing/coaching
* Experience with patients with long-term conditions, frailty, or complex needs
* Experience with undertaking home visits
* Experience with proactive care pathways or anticipatory care planning
* Previous experience of using EMIS
* Experience line managing or supervising junior staff
