We are currently seeking a motivated and GPhC-registered Pharmacist to join our friendly and professional team, supporting medication management across all clinical departments.
Key Responsibilities
* Carry out medicines reconciliation and screening of drug charts on the ward.
* Counsel patients on TTOs (To Take Outs) and medication changes to ensure safe discharge and full understanding.
* Provide accurate and evidence-based medicines information, responding to clinical queries from healthcare professionals.
* Aid and support the hospital’s antimicrobial stewardship programme.
* Carry out regular audits to maintain high standards of medication safety and compliance.
* Collaborate closely with the wider clinical team to promote safe, effective, and personalised patient care.
What You’ll Bring:
* GPhC registration as a qualified pharmacist is essential.
* Previous hospital experience is desirable but not essential – full training and development are available.
* Excellent communication and organisational skills.
* A proactive, patient-focused approach with a passion for clinical pharmacy.
