This role involves providing specialist pharmaceutical care, ensuring the safe and effective use of medications, and supporting multidisciplinary teams to optimise patient outcomes. Your expertise will be essential in medication management, clinical reviews, and patient counselling, contributing to high-quality healthcare delivery. Key Responsibilities As a Pharmacist, you will be responsible for reviewing and optimising medication regimens, ensuring adherence to treatment protocols and best practices. You will provide expert pharmaceutical advice to doctors, nurses, and other healthcare professionals, contributing to patient care plans and ensuring safe prescribing. Additionally, you will conduct medication reviews, monitor drug interactions, and support patients in understanding their treatment plans. Your role will also involve participating in ward rounds, ensuring compliance with regulatory standards, and contributing to the development of clinical guidelines. Qualifications & Experience GPhC registration (essential). Degree in Pharmacy or equivalent. Proven experience in hospital pharmacy. Strong communication and problem-solving skills.
