Job Overview
We are looking to recruit an enthusiastic Clinical Lead to join the Princeway Community Care Team in the Frodsham area. You will be joining a multidisciplinary team including Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses, AHPs and many other specialist professionals.
The role will provide confident support and clinical leadership to clinicians, working collaboratively with other professionals. Excellent communication skills, ability to work with patients and families, and safeguarding when necessary are essential. The service runs 8am‑8pm, 7 days a week, with expectation to participate on the rota until 8pm.
Main Duties
* Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload.
* Collaborate with health, social care and third sector partner agencies to meet patient needs.
* Ensure patients receive the right care, by the right person at the right time and place.
* Place patients at the centre of all decisions, streamlining journeys and encouraging independence while improving quality of care.
* Provide direction within the caseload, considering current health and social care agendas and local initiatives.
* Stay aware of local public health community profiles and their impact on required care.
* Understand skills required from the Community Care Team based on community profiles.
* Focus on delivery of person‑centred care and effective, evidence‑based care to a defined population.
* Monitor care standards through case load management and provide clinical leadership.
Essential Qualifications
* First‑level registered general adult nurse.
* Evidence of teaching and assessing.
* SPC qualification in District Nursing or equivalent, demonstrating robust evidence of caseload management, community profiling and health needs assessment.
Knowledge and Expertise
* Proficiency in delivering high‑quality evidence‑based care.
* Demonstrated continuing professional development.
* Ability to work and communicate effectively within an MDT setting.
Experience
* Significant experience working within a community healthcare setting.
* Experience of making autonomous decisions.
