About the Advice and Wellbeing Hub

The Hub is the first point of contact for adults, carers, and professionals seeking advice, support, and direction. We offer short-term intervention, designed to maximise choice, control, and independence.

Our practice is driven by the Conversations Count strengths-based model, ensuring every interaction is person-centred and outcomes-focused.

You’ll work with adults with a wide range of needs, including:

* Older adults
* People with disabilities
* Individuals affected by alcohol or substance misuse
* Homelessness
* Hoarding
* Carers seeking advice, support, and guidance

The Hub is busy, vibrant, and incredibly supportive—it’s the perfect place to grow, develop, and make an immediate impact in people’s lives.


About the Role

As a Social Worker in the Hub, you’ll be the welcoming, knowledgeable first point of contact for adults, carers, and professionals.

Your role will include:

* Completing strengths-based Care Act assessments
* Undertaking Mental Capacity assessments
* Coordinating best interest decisions
* Managing safeguarding enquiries where needed
* Offering clear, practical advice, information, and guidance
* Managing a short-term, varied caseload designed to boost wellbeing and independence

You’ll work closely with colleagues, health partners, and external agencies using your professional judgement within legal and statutory frameworks to ensure interventions are safe, effective, and proportionate.

Your contribution will be pivotal in helping people access the right support at the right time.


Qualifications

You will need to have:

* A Social Work degree (or equivalent)
* Registration with Social Work England
* A solid understanding of the Care Act 2014

You will have access to our Learning and Development programme and be supported in your career and personal growth with opportunities to learn and develop new clinical skills, helping you to build your Continuing Professional Development portfolio, extending your skills and supporting you to grow as a social worker.

We work closely with our colleagues in Reading Borough Council, Health Professionals, and Voluntary and Charity colleagues to facilitate the best possible outcomes for those people we support.

You’ll be part of a team that relies on each individual to play their part to the best of their ability. As well as the colleagues that you work closely with every day, you’ll also be part of Team Reading, playing your personal part in making Reading a successful and vibrant place.
