Job summary

An Advanced Clinical Practitioner (ACP) in GP practice provides high-level, autonomous care, including assessing, diagnosing, and treating patients with acute or complex conditions. Working within a multidisciplinary team, they conduct clinical triage, home visits, and manage long-term conditions, prescribing medications, onward referrals to secondary care.

The role will primarily be based at Angmering Medical Centre, there may be the need to cover at our other locations from time to time.

All normal duties and responsibilities associated with an Advanced Practitioner/Paramedic working within primary care.

We have a fantastic opportunity for an Advanced Practitioner (Paramedic or Nurse).

The practices are situated in great location on the Sussex Coast. Easily commutable being only a 10-20 minute drive from Worthing, 30-minute drive from Chichester and 45-minute drive from Portsmouth. There are also great transport links to London.

The practices have approx 29,300 patients and holds an abundance of knowledge with a team of a 5 Partners, 14 Salaried GPs and a supportive multi-disciplinary team which includes an experienced and established nursing team, prescribing paramedics and large prescribing team with pharmacists and technicians and physiotherapists

As a training practice (registrars and medical students), we have a strong education focus, placing high value on training and development. We have regular practice learning sessions and support our teams in their professional development and special interests.

This role is a great opportunity for an experienced Advanced Practitioner or Advanced Care Practitioner.

Job responsibilities

* Provide advanced clinical care through telephone triage, remote consultations, and face-to-face assessments for patients with undifferentiated and urgent health needs, including paediatrics, elderly care, and mental health.
* Diagnose and manage complex cases using advanced assessment, examination, and clinical reasoning skills.
* Order and interpret investigations (laboratory and radiology) and make evidence-based decisions on care management.
* Coordinate care across services, acting as a referral agent and care coordinator to ensure seamless patient journeys.
* Lead on clinical quality and safety, promoting best practice, supporting audits, and contributing to service improvement initiatives.
* Mentor and support colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students.
* Safeguard vulnerable individuals, applying policies and legislation in child protection, mental health, and safeguarding adults.
* Maintain an up-to-date professional portfolio to evidence ongoing advanced-level competency

Qualifications


Essential

* Professional registration (NMC, HCPC, GPhC) with post-qualification experience.
* Driving licence and own transport to facilitate home visits
* Advanced assessment and minor illness qualifications (Level 6 or 7).
* Proven experience in autonomous practice
* Strong communication skills, able to manage complex and sensitive situations.
* Ability to prioritise, problem-solve, and make decisions under pressure.
* Commitment to continuous professional development and reflective practice.
* Remote consultation experience.


Desirable

* Independent prescribing qualification, or working towards
* Experience in chronic disease management.
* Experience with Frailty living in the community and care and Nursing homes
* Teaching qualification
