Job Description
The Speech-Language Pathologist is responsible for comprehensive assessments in the patient\'s home, including the completion of skilled documentation through Homecare Homebase.
* Evaluate patients and perform appropriate therapy interventions
* Develop a comprehensive plan of care and coordinate multi-disciplinary care
* Perform clinical documentation and visit notes
* Continued communication with physician, care team members, patient and family
* Comfortable working in all aspects of the home health setting
* Implement and maintain SLP protocols
* Manage patient treatment and outcomes directly
Requirements
* Current Texas Speech-Language Pathology License
* One year of SLP experience
* Experience with Homecare Homebase is helpful, but not required
* Previous Home Health experience is helpful, but not required
* Current Texas driver\'s license
* CPR certification
