Main duties of the job
1. Provide clinical pharmacy services to designated inpatient wards in line with local and national standards.
2. Participate in consultant ward rounds/MDTs to improve medicines optimisation.
3. Assist and support decisions made by clinicians by providing up-to-date evidence-based information to allow informed choices to be made.
4. Provide medicines information to patients and carers to educate them about their medicines in order to help improve concordance and medicines adherence.
5. Support ward staff to ensure that adequate ward pharmacy stock levels are maintained on allocated wards and to regularly review and adjust levels if necessary, depending on prescribing trends.

Person Specification
Experience
* Experience of working in mental health Multi-Disciplinary Teams.
* Previous hospital experience is essential.
Qualifications
* GPhC Registration.
