Overview
We have an exciting opportunity for a qualified Advanced Clinical Practitioner (ACP) to join our supportive, dynamic Crisis Response Teams in Trafford or South Manchester. The Crisis teams are part of our Trafford and South Manchester Community Response Services (MCR/TCR) and launch into the new Urgent Care and Specialist Services Division on 6 October 2025.
Key Responsibilities
* Autonomous practitioner working in the Community to take an active role in responding, stabilising and facilitating safe elements of acute care pathways by delivering advanced assessment, management and treatment of patients over 18 years of age referred to MCR/TCR services.
* Provide advanced clinical assessments, history taking, diagnosis, plan and deliver treatment, prescribe and evaluate.
* Coordinate the delivery of urgent health care needs of acutely unwell people at risk of hospitalisation or requiring a facilitated timely discharge and support a more intensive level of care in the intermediate care environment.
* Work across primary, secondary and social care interfaces in Trafford or South Manchester, including direct referrals from NWAS, acute and primary care.
Qualifications & Experience
* MSc in Advanced Clinical Practice.
* Qualified Nursing or AHP (Physiotherapy or Occupational Therapy) professional background.
* Experience working in community or crisis response settings (preferred).
* Strong clinical assessment, prescribing and communication skills.
