Main duties of the job

KeyResponsibilities

* Deliver high-quality clinical care within your scopeof practice.
* Manage patient consultations and follow-ups with apersonalised approach.
* Collaborate with multidisciplinary teams includingcommunity mental health, social care, pharmacy, and other NHS serviceswithin the PCN.
* Participate in specialised clinics such as diabetesand paediatrics as required.
* Support chronic disease management
* Maintain accurate patient records and ensurecompliance with NHS standards.

Location: Westminster, LondonReports to: Practice Manager / GP PartnersHours: Part time to full time hours, flexible but minimum 2 days perweekSalary: Dependent on experience

About Us

We are a long-established NHS GPpractice located in the heart of Pimlico, serving over 14,000 patients from theWestminster area. Our diverse population is supported by a multidisciplinaryteam that provides a wide range of healthcare services including chronic diseasemanagement, paediatrics, diabetes clinics, and a responsive home visitingservice.

We are an active member of the South Westminster Primary Care Network (PCN),working closely with local NHS and voluntary sector partners to improve access,continuity, and the quality of care across our community.

RoleOverview

We are seeking a forward-thinking andcompassionate Advanced NursePractitioner (ANP) to join our dedicated clinical team. The successfulcandidate will deliver high-quality, patient-centred care, supporting bothurgent and routine needs. Youll work autonomously and collaboratively within asupportive multidisciplinary environment, with access to continuingprofessional development and opportunities to help shape service deliverythrough the PCN.

KeyResponsibilities

ClinicalDuties

* Assess, diagnose, plan, and manage care for patientspresenting with undifferentiated and undiagnosed conditions.
* Deliver same-day/urgent care consultations, managingacute presentations and minor illness.
* Lead and support in long-term condition management(e.g., diabetes, hypertension, asthma), aligned with QOF and local healthpriorities.
* Conduct telephone and face-to-face triage, ensuringappropriate prioritisation of care.
* Prescribe safely and appropriately within scope ofpractice.
* Order diagnostic investigations, interpret results,and make referrals as needed, following local guidelines.
* Participate in and support delivery of specialisedclinics (e.g., diabetes, paediatrics) and home visiting when required.
* Work as an integral part of the multidisciplinaryteam, including GPs, nurses, healthcare assistants, pharmacists, socialprescribers, and mental health professionals.
* Contribute to joint working initiatives across theSouth Westminster PCN to deliver proactive, integrated care.
* Discuss complex or secondary care referrals withsupervising GPs where necessary.

Administrative& Quality Responsibilities

* Maintain accurate and comprehensive clinical recordsusing SystmOne
* Support the practice in meeting QOF and PCN targets by opportunistically coding and recallingpatients where appropriate.
* Participate in audits, quality improvementinitiatives, and clinical meetings.
* Contribute to the development and review of clinicalprotocols and pathways.
* Support the team in embedding best practices andinnovation in care delivery.

Training,Supervision & Development

* Maintain NMC registration and complete revalidationas required.
* Participate in clinical supervision, appraisal, andtraining programs provided internally or via the PCN.
* Attend continuing professional developmentactivities, study days, and clinical updates to ensure continuedprofessional growth.
* Develop and maintain a personal development plan andreflective portfolio.

GeneralPractice Responsibilities

* Uphold patient confidentiality and data protectionin line with GDPR and NHS standards.
* Promote and support health promotion, healthliteracy, and patient empowerment.
* Contribute to a positive team culture that valueslearning, inclusivity, and innovation.
* Comply with health & safety, safeguarding,infection control, and all other relevant clinical governance standards.
* To support with the care of long term healthconditions
* Attend clinical meetings

PersonSpecification

The ideal candidate will be anexperienced, qualified Advanced Nurse Practitioner who can work bothindependently and collaboratively as part of a multidisciplinary team.

* Registration with the Nursing and Midwifery Council (NMC) as a registered nurse.
* Completion of a Masters level qualification in Advanced Clinical Practice orequivalent.
* Holds the IndependentPrescribing (V300) qualification.
* Proven experience working in a primary care or similarcommunity-based healthcare setting.
* Demonstrated ability to assess, diagnose, and managepatients with minor illness,urgent care needs, and chronic conditions.
* Excellent communication and consultation skills,with a compassionate and patient-centred approach to care.
* Confident in autonomous decision-making and safe clinicaljudgement.
* Commitment to ongoing professional development and a strong understandingof clinical governance and NHS standards.
* Experience managing patients with long-termconditions such as diabetes,asthma, or COPD.
* Familiarity with SystmOne and digital triage tools (e.g., AccuRx).
* Prior involvement in Quality and Outcomes Framework (QOF) delivery or PCNinitiatives.
* An understanding of integrated care pathways andexperience working within a PrimaryCare Network (PCN) or alongside community partners.
