In March 2023, St Elizabeth Hospicemerged with East Coast Hospice to form St Elizabeth East Coast Hospice, withall service provision in Great Yarmouth and Waveney coming under the StElizabeth East Coast Hospice name.
Thisis an exciting time for St Elizabeth East Coast Hospice, as we are developingan achievable and sustainable palliative and end of life care service for theresidents of Great Yarmouth and Waveney. Its a time of expansion, developmentand investment in the Hospice Team. We areoffering an opportunity to be part of the Community Specialist Palliative CareTeam and also work within our Community Care Unit influencing the holistic careof patients and carers with end of life and specialist palliative care needs. Main duties of the job
We arelooking for a compassionate and experienced Registered Nurse to join our dedicatedCommunity Team to support informal carers. This exciting role willwork within our multi-disciplinary team to provide guidance, emotional supportand practical advice to unpaid carers and families looking after loved oneswith specialist palliative care needs. You will play a key role to ensuringinformal carers feel supported and empowered to navigate care responsibilitieswhilst maintaining their own wellbeing. Youwill have a passion for supporting carers and improving outcomes for peopleproviding unpaid care in the community.
St Elizabeth Hospice continues to driveforward innovation and development of services, both in the Ipswich and EastSuffolk locality and further up the coast to the Great Yarmouth and Waveneyarea. In 2019 NHS Norfolk and WaveneyCCG awarded the specialist palliative and end of life care contract to EastCoast Community Healthcare. The contract includes a specialist palliativecare service which will be delivered in partnership with St Elizabeth Hospice. Our most recent Care Quality Commissionreport rated the hospice as Outstanding.

Job responsibilities
JOBSUMMARY
To provide informalcarers, who are caring for someone known to the hospice, with support andclinical advice where there are complex needs or experiencing crisis. To workat an enhanced level of practice to manage community patients in complex,challenging and changing situations and environments, seeking further guidancewhen the boundary of competence is reached. Will possess skills and capabilities acrossall four pillars of nursing: clinical practice, education, research andleadership. Required to function to a high level of autonomy within specialistpalliative care.

KEYACCOUNTABILITIES
To promote informalcarer wellbeing to carers of known hospice patients
Provide person centredcare to informal carers, identifying and addressing their health-related needs
To embed carersassessments throughout the service and complete a thorough assessment of needthrough the Carer Support Needs Assessment Tool Intervention (CSNAT-I)
To provide short-termcase management for informal carers with complex health heeds and who presentin crisis
Provide advice andresources to other members of the team in relation to identifying andsupporting informal carers
To undertake in-depthholistic assessments of patients with palliative care needs, regardless oftheir diagnosis and plan care to meet patient outcomes.
To provide information,advice, psychological and spiritual care to patients, their families andcarers, liaising and referring on as necessary to appropriate healthcareprofessionals in the Hospice, Hospital and community.
Responsiblefor and manage a caseload of patientsoffering continuity of care.
Work in acollaborative, compassionate way, promoting equality, respecting diversity, andensuring inclusivity, taking responsibility for challenging behaviour that doesnot align with these values.
Apply clinicalreasoning to individual presentations and complex situations, utilisinginformation from different sources to make timely, appropriate, andevidence-based judgements.
Optimise safety throughassessment and management of risk.
Act as a key role within the PrimaryCare Hubs (PCH) taking a population health approach to assist in the managementof carers in the end-of-life population within the PCHs.
Use advanced communication skills indelivering highly emotive information clearly, sensitively and unambiguouslywith staff, patients, families and professionals by face to face, telephoneand/or electronic means. This may bewhere there are barriers to understanding.
Provide skilled and expert palliativenursing interventions, working closely with medical and other colleagues toensure a coordinated and effective service across boundaries of inpatient,community and outpatients and across professionals.
Maintain and develop workingrelationships with community and hospital colleagues ensuring effectivecommunication so enhancing patient care
Involve patients,families, and carers in co-production of and making informed decisionsregarding strategies to manage their own health and wellbeing.
Participate in the developmentand updating of practice policies/guidelines and procedures using high-qualitycontemporary evidence-based resources.
Participate in the evaluationof significant events, capturing the learning from these experiences to improvepatient care and service delivery.
Identify own personaldevelopment needs, proactively seek feedback, and take effective action toaddress them using a mix of strategies.
Share knowledge,experience and expertise with nurses and others at all levels, throughstrategies such as formal and informal teaching, facilitated reflectivepractice, skills supervision, coaching, mentoring and preceptorship.

Person Specification
Qualifications
* First level Registered Nurse with current NMC registration.
* Level 6 and / or level 7 post registration education in palliative care or related qualification.
* Advanced communication course.
* Non -Medical Prescribing.
* Counselling qualification.

* Expert current knowledge of pain and symptom management, which underpins theory, usually gained through recent clinical practice at a senior level.
* Ability to identify psychological or spiritual distress requiring referral to other specialist services.
* Evidence of counselling skills and practical application.
* Evidence of ability to interact and influence at all levels within the organisation.
* Commitment to clinical excellence through regular professional updating.
* Computer literacy sufficient to input patient records and work efficiently and effectively.
* Able to prioritise and manage own time effectively.
Personal Qualities

Experience
* Significant post registration experience including proven palliative care / Oncology experience at a Band 6 or above, or equivalent level.
* Experienced in assessing patient and carer needs that may be complex and subject to change.
* Knowledge of the national palliative care agenda.
* Have an understanding of safeguarding principles and be able to apply these within everyday practice.
* Experience of working in the community.
* Previous experience of developing and implementing policies and procedures.
