Job Summary
To work closely with ward staff involved in discharge planning to ensure that the assessments and referrals for services relating to the patients discharge are completed comprehensively and in a timely manner. This will involve patient‑facing contact.
Main duties
* Work closely with ward staff involved in discharge planning to ensure that assessments and referrals for services relating to the patient’s discharge are completed comprehensively and in a timely manner.
* Ensure the delivery and maintenance of a high standard of care that is patient centred and evidence based.
* Work with teams and specialties across the system to facilitate safe discharges in a timely manner.
* Adhere to the NMC Code of Professional Conduct, Guidelines and Standards and ensure colleagues do too.
* Provide high standards of evidence-based patient‑centred care and knowledge for all patients, respecting dignity and privacy.
* Deliver high quality care for patients within the discharge process and ensure all discharge plans are robust to provide a seamless and cohesive transition to other settings.
* Attend mandatory training.

Person Specification
Essential
* Registered Nurse or equivalent health care professional
* Significant relevant post‑registration experience
* Knowledge of the Mental Capacity Act, Safeguarding Adults and Deprivation of Liberty
* Recent and significant experience of working with patients/carers/MDT in preparing for discharge from hospital
* Evidence of an ability to maintain effective working relationships
* Evidence to implement and facilitate change at departmental and organisational level
Desirable
* Evidence of CPD
* Ability to present and teach
* Recent experience of working across primary and secondary care
* Specialist knowledge of the discharge to assess model and discharge pathways
* Experience of producing and assimilating reports using a variety of data systems
* Ability to work flexibly in order to meet the service/organisation requirements
* Demonstration of excellent written and verbal communication skills across all levels
