Community Mental Health Nurse (Social Worker)

Blackpool Community Mental Health Team is seeking to appoint a Band 5 Community Mental Health Nurse/Social Worker.
This role will be responsible for delivering holistic care to patients in a community setting including homes and clinics. Therefore you must be able to manage the travel requirements of the role to deliver a comprehensive service.
Main duties of the job
We are seeking a motivated and compassionate band 5 community nurse or Social Worker to join our dynamic community healthcare team. You will provide high quality patient cented care - promote independance and support patients with a wide range of health needs

Job responsibilities
You will develop a broad and valuable clinical skill set, managing diverse conditions and working autonomously while being supported by the multidisciplinary team.

Person Specification
Qualifications
* Registered Mental Health Nurse Degree/Diploma or Social Worker qualification
* On-going registration with the NMC/SWE.
Experience
* Able to work as part of a multidisciplinary team
* Experience of assessing, planning and implementing and reviewing care needs
Knowledge
* Understanding of Health and Safety policies and procedures
* Knowledge and practice of Mental Health Act 1983
* Knowledge and practice of the Mental Capacity Act 2005
* Can provide evidence of recent professional/personal development
* An understanding and awareness of the NHS Plan, and other relevant documentation
* Knowledge of common and complex mental health problems and medication used in relation to these
