The relocation package is for anyone who is joining CWP in a clinical role at Band 5 or above, who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.
An exciting opportunity has arisen for a highly motivated & dynamic nurse to join Wirral All Age Continuing Care & Complex Care Service for a Specialist Children’s commissioning nurse, within CWP.
This is a role within a skilled, supportive, and welcoming team to join an existing member of staff and work together in managing the caseload.
The successful candidate should have experience of undertaking complex assessments, and working with patients, families, & carers. They should have a sound knowledge of the NHS Framework for Continuing Care with demonstrable experience of implementing the National Framework and commissioning for patients which may include via a Personal Health Budget.
The successful candidate will be responsible for the assessment process from referral to assessment, review, care commissioning and case management for children and young people with Continuing Care needs.
Main duties of the job
Partnership working with adult clinical commissioning colleagues specific to Physical Disabilities, Learning Disabilities and Mental Health is a key requirement of the role. Knowledge or experience in children’s and young people mental health or CAMHS would be welcomed.
The successful candidate will be responsible for leading on the transition process for CYP on the caseload into adult services. They should have excellent leadership and communication skills with the ability to manage challenging situations in a professional and effective manner.
Assessment, Review, Commissioning and Case Management for Children and Young People with eligible needs.
To meet individual ongoing health care needs of these clients and hold responsibility for coordinating the process of securing health funding whether it is Continuing Healthcare or Joint funding arrangements with the Local Authority.

Access to a vehicle for business purposes is essential. Whilst the role is based on the Wirral, there may be times when it is necessary to travel further afield as part of the role to see patients placed in other areas of the northwest / country.

Qualifications
Essential criteria
* Degree or equivalent level of experience in healthcare
* Evidence of professional development NMC registration – Registered children's nurse

* Relevant post basic academic / professional qualification at higher degree level or equivalent

* Demonstrable relative experience and knowledge of current key policies and priorities
