Job overview

The Out of Hours Urgent Community Response (OOH UCR) team combines the patients traditionally seen by both District Nursing and our Intensive Home Support Service. As the team is still in its infancy, this is an exciting time to join the service as we come together and embed the level and breadth of care and treatment we are able to deliver to our patients in their own home, preventing unnecessary hospital admissions and meeting the national guidance around Urgent Community Responses.

A permanent Band 7 position is available for an enthusiastic and dedicated registered nurse to support the leadership of this service. For the successful candidate this is an excellent opportunity to work in a supportive environment as a Team Leader, developing high level skills in patient assessment and care and contributing to leadership within the team.

The team works between 21:00 and 08:00 every night and cover a Pennine Lancashire footprint. Occasional day time work will be required to meet your learning and development needs. This a 7-day service, including weekends and Bank Holidays. Operating across our two bases at Burnley General Hospital and St Ives House, Blackburn, the post holder must demonstrate professional standards of practice and personal qualities and behaviours as expected by the NMC Code and be passionate about high quality patient care.


Main duties of the job

Lead on the co-ordination and delivery of care in line with national and local guidance.

* Assessment of new and complex patients
* Delivery of comprehensive Senior Reviews of patients in their own homes and other community care settings
* Working as part of a wider multidisciplinary team
* Supporting students and new staff in learning and development in the service
* Development of the team through leadership and role modelling
* Management of staffing rotas and allocation of clinical work
* Establishing and maintaining effective communication with all clinical staff, patients and families
* You will have the ability to work contracted hours rotationally across day and night shifts, as reasonably required by the needs of the service.

The post holder must demonstrate professional leadership and be passionate about patient care. They will need to be able to problem solve and adapt to challenging situations, manage resources efficiently and ensure the delivery of safe, personal and effective patient care. We are looking for a nurse with a commitment to quality care and the ability to positively challenge to ensure patient and staff safety.

Car driver with full UK driving licence and access to a vehicle is essential.

Qualifications


Essential criteria

* Degree in appropriate subject or evidence of education to degree level or equivalent experience
* Current NMC registration – Registered General Nurse


Desirable criteria

* Non-medical prescribing V300 or working towards
* Clinical Examination and Clinical Diagnostic CPD Modules at Masters level or evidence of working to that level or currently working towards
* Leadership module or equivalent Leadership study days or relevant experience


Experience


Essential criteria

* Evidence of clinical experience at a high level in a range of clinical settings including the community/primary care setting.
* Proven experience of working with complex cases
* Evidence of managing and co-ordinating a team
* Evidence of experience in mentoring/teaching role.


Desirable criteria

* Leading change and innovation


Work Related Knowledge and Skills


Essential criteria

* High level of clinical knowledge including current evidence-based practice
* Knowledge of chronic disease management


Personal Attributes


Essential criteria

* Self-confidence to challenge traditional practice and the persistence to address difficult, enduring issues.
* Self-motivated and adaptable with changes being implemented
* A current driving licence
