Role Purpose The Care Co-ordinator plays a central role in ensuring the smooth and effective delivery of high-quality domiciliary and community-based care services. Responsible for care planning, staff allocation, rota management and communication. Key Responsibilities 1. Care Assessments and Planning Support assessments for new and existing service users. Maintain and update personalised care plans. 2. Staff Scheduling and Rota Management Build and maintain rotas. Respond to staff absences and ensure continuity. 3. Service Delivery & Operational Support Ensure shifts and visits run smoothly. Support the Registered Manager in daily operations. 4. Quality Assurance and Compliance Conduct spot checks, audits and client reviews. Report incidents and safeguarding concerns. 5. Staff Support and Supervision Provide communication, guidance and updates to care staff. Assist with performance monitoring. 6. On-Call Responsibilities Participate in on-call rota and emergency response. Person Specification Essential: Previous care coordination or scheduling experience. Strong organisational and communication skills. Desirable: NVQ Level 3. Experience in domiciliary care.
