Locations: Mendip, Bridgwater, Taunton. Full time, 37 hours per week. Starting salary £37,280 rising to £41,771 per annum. Hybrid working opportunities.
Responsibilities
* Work as part of a small, supportive Learning Disability and Autism service in the West or East of Somerset.
* Complete Care Act assessments, reviews, mental capacity assessments, Safeguarding Enquiries and Court of Protection work.
* Participate in a duty system across the County and use approaches such as motivational interviewing, crisis intervention and positive risk management.
* Collaborate with colleagues and specialist professionals in weekly Peer Forum discussions to contribute to decision making and requests for increases to personal budgets and packages of care.
* Work with other specialist teams (Preparing for Adulthood, Commissioning, Occupational Therapy, DOLS, Mental Health, Safeguarding, CHC) and LD Health colleagues in Somerset Foundation Trust.
* Advocate a strength-based, personalised approach using the three conversations model to help people live more independent and fulfilling lives.
* Develop knowledge of the local area and partnerships with local agencies; provide creative solutions; contribute to the development of the Adult Social Care service.
Qualifications
* Qualified Social Worker registered with Social Work England.
* Access to/use of a vehicle is an essential requirement for this role.
