We are advertising for up to 4 clinical sessions for a GP ARRS session. The GP will need to be present in the practice (this is not a remote role). The days of the week can be negotiated but ideally be available on a Thursday.
Provide high-quality, safe and patient-centred general medical services to a diverse practice population.
Undertake face-to-face, telephone and digital consultations, assessing, diagnosing and managing patients presenting with a wide range of acute and chronic health conditions.
Work collaboratively within a multidisciplinary team, including pharmacists, first contact practitioners, mental health practitioners, social prescribers, care coordinators and nursing staff, to deliver integrated and personalised care.
Support the effective delivery of the practice's total triage model, ensuring patients are directed to the right clinician or service at the right time.
Contribute to the proactive management of long‑term conditions, preventative healthcare, screening programmes and health promotion initiatives.
Participate in the review and management of complex patients, including those with frailty, multiple long‑term conditions and mental health needs.
Support quality improvement activities, clinical audits, service development and implementation of new models of care aligned with Primary Care Network and NHS priorities.
Maintain accurate and contemporaneous clinical records using the practice clinical system and work in accordance with GMC standards, national guidance and local policies.
Contribute to practice meetings, multidisciplinary discussions and shared learning activities to support continuous improvement in patient care.
