About The Role
HC‑One Scotland is developing a Specialist Dementia Care Community (SDCC) at Darnley Court care home. The SDCC will provide a bespoke living environment with dedicated calming de‑escalation rooms, enhanced staffing and specialist training for nurses and allied practitioners. The Dementia Nurse Practitioner will support, deliver and coordinate person‑centred dementia care and therapeutic interventions within this purpose‑designed setting.
Key Responsibilities
* Coordinate person‑centred dementia care and therapeutic interventions within the SDCC.
* Deliver evidence‑based psychosocial and holistic nursing assessments and interventions.
* Audit and evaluate clinical practice against relevant policy and guidance.
* Collaborate with the multidisciplinary team of nursing, social care, psychology and wellbeing practitioners.
* Implement best practice in behaviour support, de‑escalation, and safe restrictive practice.
* Maintain continuous professional development and reflective practice.
Essential Skills & Experience
* Post‑registration clinical experience of working with people with dementia.
* Experience of completing holistic nursing assessments.
* Experience of working as an autonomous practitioner.
* Evidence of delivering person‑centred care for people with complex dementia needs.
* Evidence of sharing knowledge and delivering best practice.
* Evidence of using critical reflective practice.
* Collaborative and multidisciplinary team working.
* Experience of providing general nursing and bio‑psychosocial interventions.
* Experience of working as an autonomous practitioner.
Desirable Skills & Experience
* Knowledge of current dementia policies and guidance.
* Experience of providing positive behaviour support and psychosocial interventions.
* Experience of auditing and evaluating clinical practice.
* Ability to work in intense situations requiring physical, mental and emotional effort.
