About this job
The Child Health & Disability (CHAD) Team provides specialist support to children with complex physical and learning disabilities, life‑limiting conditions, and additional needs. The team works closely with multi‑agency partners to promote independence, safeguard wellbeing, and support children through key developmental and transitional milestones.
Responsibilities
* Work alongside a multi‑agency team of professionals in partnership with families
* Use the Signs of Safety framework to underpin social work practice
* Complete Wellbeing Assessments, child protection enquiries, and develop/review care and support plans
* Attend Child Protection Conferences, Looked After Children Reviews, and court when required
* Provide specialist advice across Children’s Services regarding children with additional needs
* Support children and young people through transitional milestones
* Participate in the team duty rota and regular team meetings
* Maintain high‑quality written and verbal practice
* Provide peer support to newly qualified social workers
Candidate Requirements
* Degree or Masters in Social Work, Diploma in Social Work, or equivalent
* Experience working within a Child Health & Disability Team
* Experience working as part of a multi‑disciplinary team around children
* Strong understanding of safeguarding, disability, and statutory social work practice
* Ability to form and maintain appropriate relationships with children with complex needs
* Excellent written and verbal communication skills
* Ability to work under pressure and manage a varied caseload
* Strong IT skills and ability to maintain accurate records
