Job summary
The post holder is responsible for the delivery ofgeneral practice nursing to the whole practice population.
The focus of the role is both the delivery ofevidence-based practice for patients presenting with a long-term condition, andthe provision of preventative health care to the practice population. As anautonomous
Practitioner the nurse is responsible for the caredelivered, demonstrating critical thinking and skills in clinicaldecision-making in the management of patients. They will work collaborativelywithin the general practice team to meet the needs of patients, support thedelivery of policy and procedures, and provide nurse leadership as required.
Key responsibilities
Assess, plan, develop, implement, and evaluatetreatment programmes that promote health and well-being.
Assess, plan, implement and evaluate individual treatmentplans for patients with a known long-term condition.
Proactively identify, diagnose, and managetreatment plans for patients at risk of developing a long-term condition asappropriate.
Work with other health care professionals todiagnose, monitor, manage and treat long-term conditions, includingnon-drug-based treatment methods using a management plan, and in line withnational and local policies and practice needs.
Review medication for therapeutic effectiveness,appropriate to patient needs and in accordance with evidence-based practice andnational and practice protocols.
Work with patients in order to support adherence toprescribed treatments.
Provide information and advice on prescribed or over-the-countermedication on medication regimens, side-effects and interactions.
Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care
Support patients to adopt health promotionstrategies that promote patients to live healthily, and apply principles ofself-care.
Support and manage health needs of women presentingfor family planning and cervical cytology consultations.
Recognise, assess and refer patients presentingwith mental health needs in accordance with the NSF for Mental Health
Implement and participate in vaccination andimmunisation programmes for both adults and children.
Advise, support and administer vaccinations whereappropriate for patients travelling abroad.
Promote and deliver evidence-based care forpatients presenting with aural conditions.
Meet the needs of patients presenting foropportunistic wound care.
Leading all aspects of infection-control and applicationof practice protocols related to this area of Health and safety .
