Job responsibilities Key duties and responsibilities include: Clinical Practice Provide high quality and current evidence based medical care to the patients of the Indigo Gender Service. Actively participate in the development of the service and relationships with existing services to provide a holistic, community focused service that meets the needs of a diverse group of patients. Holistically assess patients to help create an individualised and specific treatment plan. Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. You will be skilled in particular clinical areas that complement the General Practitioners, to include contraception, fertility, screening etc. You will develop your skills and competence alongside other clinicians. To support a level of continuity of care within a service that helps reassure patients and provide a degree of clinical input. To support the longer term follow up of patients that are already established on a medical treatment pathway that have not yet been discharged back to the care of their GP. Communication Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Anticipate barriers to communication and take action to improve communication. Maintain effective communication within the organisation and with external stakeholders. Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate. Non-Medical Prescribing If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with NMP governance registration processes. You must maintain your knowledge and skills and prescribe within your clinical competencies. The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance. Delivering a Quality Service Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Support and undertake clinical audits as required. Ensure professional revalidation requirements are met. Deliver care according to Royal College of General Practitioners (RCGP), NSF, NICE guidelines and evidence-based care. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities. You will gain and maintain competency through continued professional education, reflecting on lessons learnt and any change in practice based on this. It will be a requirement that a portfolio of supporting information is kept that demonstrates the specific requirements and an annual appraisal will be undertaken with your clinical supervisor. In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate. Support and participate in shared learning across the service and wider organisation. A commitment to undertake the RCP training via the professional certificates for gender identity professionals (funded by NHSE).
