To work as part of the Interdisciplinary Team (IDT) that includes Physiotherapists, Occupational Therapists, Speech and Language Therapists, Dieticians, Clinical Psychologists, and Neuro Specialist Nurses, to assist with patient assessment and rehabilitation for various neurological conditions. The role involves promoting independence and wellbeing in community settings, including homes and social environments, through a holistic approach respecting patients' values and beliefs.
The post requires independently executing treatment plans set by qualified clinicians, using a competency-based practice to complete subjective, objective, and analytical components of rehabilitation. The SRF will be integral to the team, discussing patient progress and treatment adjustments. The role also involves monitoring responses, providing feedback, and working without direct supervision while ensuring all interventions are documented accurately.
Key responsibilities include supporting clinicians with treatment programs, promoting health initiatives, assisting with group sessions, guiding families, and documenting patient progress. The post also involves participating in team meetings, contributing to service models, managing caseloads, and maintaining professional development through ongoing training and supervision.
The role demands excellent communication skills, the ability to establish therapeutic relationships, and the flexibility to work extended hours, including weekends. A minimum of NHS experience and either an RQF Level 2 NVQ in Health & Social Care or the NHS Care Certificate is required. This is a full-time, permanent position (37.5 hours, Monday to Friday).
