An exciting opportunity has arisen to become a community staff nurse and join the Aylesbury Adult Community Healthcare Team.
The post is to provide nursing interventions to patients in their home environment. If you are interested in joining our friendly team we would like to hear from you.
Participate in weekend rotas / 7 day working rotas
Main duties of the job
1. To contribute to processes for service and quality improvement
Assess the physical and psycho–social needs of patients and deliver therapeutic interventions based on best available evidence in order to improve health outcomes.
2. To manage own case load, delivering and evaluating service delivery to patients with a variety of medical conditions and those delegated by case managers.
3. To plan interventions with the Case Manager and other team members using the common assessment process
4. To take part in the referral management process to ensure patients are treated by the most appropriate member of the team/service
5. To contribute to the development of protocols and procedures developed for clinical pathways
6. Assist in the development of case management protocols and guidelines within their own area of professional practice.

SKILLS, ABILITIES & KNOWLEDGE
Essential criteria
16. Good verbal and written communication skills and ability to negotiate
17. Ability to manage and prioritise own caseload with supervision
18. Ability to solve problems and use initiative
19. Ability to develop clinical skills
20. Willingness to undergo training (e.g. medications)
21. Basic IT skills
22. Knowledge of the NMC code of conduct
23. Ability to maintain a safe clinical environment conducive to patient care Adaptable to change
24. Ability to work rotating shifts
25. Diplomatic approach with professionalism
26. Flexible and approachable
Desirable criteria
27. Willingness to develop teaching skills
28. Awareness of the NHSE guidance of clinical governance in order to continuously improve quality of care e.g. quality, safety and ward audit standards
29. Knowledge of care planning, preceptorship, reflection and clinical supervision

EDUCATION, QUALIFICATIONS & TRAINING

35. Registered Nurse/Awaiting NMC PIN
Desirable criteria
36. Evidence of further continued professional development
37. Post qualification experience in relevant speciality
38. Hold / be working towards an NMC approved mentoring qualification
