Job overview
The post holder is responsible for the delivery of basic practice nursing services care to the practice population. Supported by senior nurses within the Practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required.
Main duties of the job
1. Assess, plan, develop, implement and evaluate programmes to promote health and well-being and prevent adverse effects on health and well-being
2. Implement and evaluate individual treatment plans for patients with a known long-term condition
3. Assess and care for patients presenting with both simple and complex wounds
4. Support and advise women requesting information relating to family planning needs
5. Support and manage health needs of women presenting for cervical cytology consultations
6. Implement and participate in vaccination and immunisation programmes for both adults and children
7. Advise, support and where appropriate, administer vaccinations for patients travelling abroad

Essential criteria
8. RGN with current NMC registration Evidence of Continued Professional Registration
9. Recognised qualifications in diabetes or chronic heart disease or respiratory medicine
10. A diploma in General Practice Nursing
11. Previous Experience in Primary Care
12. Qualifications/Certificates in Cytology, Travel vaccinations, Immunisations
Experience

13. General nursing experience
14. Working within Primary Care setting
15. Nursing management of chronic disease conditions in general practice, i.e. Diabetes, Asthma/COPD, Heart Disease
Knowledge and Skills
