As a registered nurse with interest in NHS Continuing Healthcare and Funded Nursing Care, you will join the busy AACC team delivering all aspects of the CHC framework.
This job helps improve outcomes for patients, service users and populations by:
Applying the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (NF) in a fair, consistent and robust way by completing robust CHC assessments for all Kent and Medway residents and ensuring the safe and effective care delivery for CHC eligible patients in line with the organisations commissioning function.
Main duties of the job
Under the guidance and support of the Clinical Lead, the Nurse Assessor will assess individuals for NHS Continuing Healthcare. Applying the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (NF) in a fair and consistent way by completing robust CHC assessments for all Kent and Medway residents and ensuring the safe and effective care delivery for CHC eligible patients in line with the organisations commissioning function and the National Framework for NHS Continuing Healthcare and NHS Funded Nursing care, revised July 2022.
