Primary Responsibilities:
1. Provides patients, families and caregivers information about Community Hospice services and philosophy and other community related resources as appropriate.
2. Assists with obtaining Community Hospice & Palliative Care consents.
3. Provides information regarding Health Care Surrogate, Health Care Proxy and other Advance Directives.
4. Assists as directed in arranging/coordinating and supporting patient care at the time of admission and/or when discharging from one care location to another.
5. Documents patient/family contacts
6. Establishes, develops, and maintains relationships with area social workers, case managers and community agency staff in order to network and facilitate admission and placement of patients.
7. Provides emotional support to family for patients in active dying process. May make bereavement calls for patients who die while hospitalized.
8. Attends in-service trainings, workshops, conferences, agency and departmental meetings.
9. Complies with appropriate professional code of ethics in delivering services to patients and families.
10. Other duties as assigned.
Required Licenses/Certifications:
Current State of Florida driver’s license with reliable transportation and driving record acceptable to company insurance carrier.
Qualifications:
11. Minimum of bachelor’s degree in the field of case management, social work or related field or licensed nurse (RN/LPN). Three (3) years of related healthcare experience may substitute for above minimum requirements.
12. Basic proficiency utilizing Microsoft Office software products
Knowledge of:
13. Hospice services and philosophy.
14. Admission procedures.
15. Medicaid Application Process.
16. Community resources.
17. Referral and placement protocols for Independent Living, Assisted Living and Long Term Care facilities in the service area.
18. Must be willing and able to flex hours and access all locations of care facilities including hospitals and other health care facilities in order to meet admission and referral requests.
19. Must possess excellent verbal and written communication skills; strong critical thinking ability.
20. Ability to work within a framework of multidisciplinary teams
21. Ability to work in stressful and sensitive environments while maintaining professional demeanor with patients, families and referral sources
22. Ability to research, analyze and interpret information
23. Ability to organize and prioritize work schedule
24. AHCA Level II Background Screening required
