Capable of effectively managing a caseload and building strong relationships with both clients and carers to ensure comprehensive care and compliance with key performance indicators (KPIs). What the Job Involves This role entails coordinating care for clients, including frequent follow-ups and monitoring of mental health states. You will work to maintain up-to-date KPIs, support carers, and ensure proper linkage with agencies inside and outside the NHS and social care systems. Responsibilities also include performing Care Act Assessments, carer assessments, social circumstances reports, and coordinating with MHAA/MCA. Additional support tasks as required by the team are also part of this role.
