Role Overview
We are looking for an experienced Assistant Practitioner to join our innovative Integrated Care Team of Community Nurses, Advanced Nurse Practitioners, Healthcare Assistants, Social Care, and Allied Health Care Professionals. In this generic role, you will collaborate closely with a range of services and care for a diverse group of patients, helping deliver the right care, in the right place, at the right time. We prioritise flexible, agile working while maintaining high‑quality care, seven days a week.
Responsibilities
* Collaborate closely with a range of services to support community patient care.
* Deliver care in the right place and at the right time, ensuring patient safety and satisfaction.
* Work a variety of shifts, including weekends, with hours Monday to Sunday 08:00–20:00.
* Provide supportive induction and ongoing development opportunities as part of the team.
* Operate within a flexible, patient‑centred environment to reduce hospital visits and embrace digital solutions.
Qualifications & Experience
* Previous experience as a support worker.
* Excellent verbal and non‑verbal communication skills.
* Leadership qualities and the ability to work well within a team.
* Evidence of continuous professional development and broad clinical knowledge.
* Ability to work well in a high‑pressure environment and adapt to changing situations.
Working Hours & Conditions
Fixed term, 12‑month secondment or part‑time position. Full‑time reduction to 36 hours per week from 1 April 2026; full‑time pay will not change. Shift patterns include weekend work.
