Overview
Urgent Community Response Services across Sunderland. Recovery at Home team based in Sunderland. MDT includes nurse practitioners, occupational therapists, physiotherapists, health care assistants, therapy assistant/exercise practitioners, admin staff. Works closely with partner agencies.
Responsibilities
* Provide rapid assessment and care to patients in own homes to prevent admission/readmission.
* Undertake comprehensive holistic assessment, prioritise clinical need, initiate investigations, determine differential diagnosis, implement evidence‑based plan.
* Maintain standards of care in community setting, including bed‑based rehabilitation.
* Triage calls, determine clinical priority, coordinate team dispatch.
* Collaborate with multi‑professional teams to deliver efficient, effective, evidence‑based practice.
* Develop professional scope to meet service requirements per guidelines.
* Deliver expert knowledge to all service users.
Person Specification
Experience
* Post‑qualification experience relevant to role.
* Experience with acute or chronic long‑term conditions.
* Experience in multidisciplinary working.
* Experience supervising junior staff and students.
Qualifications
* Registered with HCPC.
* Degree.
* Willingness to undertake teaching qualification, practice educator.
