Overview
The role is based within the Hospice at Home (H@H) team, delivering 24/7 specialist palliative and end-of-life care to enable patients to be supported at home where possible, and to support carers. The team provides rapid response to facilitate discharge from hospital to home and reduce hospital admissions. The post holder will perform assigned tasks involving direct patient care under supervision of a Registered Nurse and maintain a high standard of care in the patient’s home.
What we are looking for
We are seeking motivated and passionate individuals with previous experience as a healthcare assistant. The ideal candidate will have experience working in a community setting with patients at the end of life. The role requires enthusiasm, flexibility and excellent communication skills; ability to work well within a multi-disciplinary team. The post involves shifts including long days and nights (07:30-20:00 / 19:30-08:00) and regular weekends. A full UK driving license and access to a vehicle are essential to travel across the hospice’s catchment area.
Key Responsibilities
* Deliver personalised palliative care and symptom management at home, visiting patients 24/7.
* Support the SMH Living Well Service, including MND and long-term neurological conditions.
* Maintain high standards of care, communication, and record-keeping.
* Collaborate with healthcare professionals, patients, families and carers to ensure a seamless, compassionate service.
* Assist the Registered Nurse with general and specific patient care, including practical nursing tasks and emotional support to patients and their carers.
Role details
The post holder is part of a multi-professional team within the Hospice at Home service and may support 24/7 care, including rapid response for hospital discharge and home-based care for patients in the last two weeks of life.
Qualifications and requirements
* Experience as a Healthcare Assistant; community/end-of-life care preferred.
* Full UK driving license and access to a vehicle.
* Ability to work flexible shifts, including long days and night shifts, and weekends.
* Excellent communication and teamwork skills.
