You’ll work closely with the District Nurse Locality Leads, offering strategic leadership and hands-on support to create a seamless, patient-focused service.

Main duties of the job

As a District Nursing Lead (Advanced District Nurse Specialist), you will provide expert clinical leadership and proactive management of District Nursing teams, ensuring high-quality, evidence-based care for patients with complex needs.

You’ll lead the implementation of the national anticipatory care model, champion preventative and personalised care, and work collaboratively with GPs, multi-professional teams, and locality leads to reduce hospital admissions. Your role includes advanced clinical assessment, prescribing, and developing care pathways, alongside supervising and supporting staff to deliver safe, effective care.

To succeed, you’ll need strong clinical leadership skills, advanced assessment and decision-making abilities, and a commitment to continuous improvement. Excellent communication, emotional intelligence, and the ability to manage competing priorities under pressure are essential, as is experience in change management, education, and integrated working.

You’ll be a role model for professional standards, fostering a culture of learning and quality across the service.

Detailed job description and main responsibilities

Oxfordshire District Nursing Service is forward looking and dynamic, involved in national community nursing initiatives and improving the service to meet future demand.

Career progression is established within the District Nursing Service and continues to be developed, especially around senior clinical and leadership roles.

The District Nursing Service sits within Planned and Preventative Care within the Community Services Directorate and works in an integrated way to deliver care to housebound and other patients in their own homes, clinics and residential care home.

Personal Qualities


Essential criteria

* Evidence of leading within and across teams, including multi-disciplinary and cross organisational agencies
* Facilitates and coordinates advanced levels of practice in self and others.
* Well-developed communication and interpersonal skills.


Qualifications


Essential criteria

* Advanced History taking or Comprehensive assessment of the person with frailty module
* District Nursing Specialist Practitioner Qualification (DNSPQ)
* Non-medical Prescriber (V300) or willingness to undertake module


Desirable criteria

* Post qualifying NMC approved Community Practice Teacher preparation.
* Coaching and facilitating learning module.


Knowledge


Essential criteria

* Autonomous practitioner initiating and facilitating evidence-based practice at an advanced level
* Evidence of strong clinical leadership skills
* Demonstrates an understanding of local and national issues influencing the delivery and development of health care.
* Previous experience of implementation of change


Desirable criteria

* Evidence of participation in service review and facilitating change in the delivery of clinical services.
