Work proactively to identify, address and escalate barriers to discharge
Liaise with local authorities, community teams, MAPPA, safeguarding and partner agencies
Ensure legal frameworks (MHA, MCA, Care Act) are applied appropriately
Support families and carers, ensuring their voices are heard in planning
Provide clear, timely updates to MDTs, commissioners and senior leaders
Contribute to flow‑focused improvement work across the Birmingham site
This is a role for someone who is solution‑focused, confident in multi‑agency work and motivated by improving patient journeys.
