Thepost holder will work as part of a multidisciplinary team, with their timesplit between face to face clinical reviews with patients and desk based activities according to the needs of the practice (e.g. prescription management. prescribing safety, clinical audit, local incentive scheme support, medicines reconciliation).

The postholder will act within their professional boundaries, supporting and workingalongside a team of pharmacists in general practice. In this role, they will besupported by a Lead Pharmacist who will develop, manage and mentor them.

As part of amulti-disciplinary team the post holder will take responsibility for areas ofchronic disease management within the practice and undertake clinicalmedication reviews to proactively manage patients with complex polypharmacy.

The postholder will provide primary support to general practice staff with regards toprescription and medication queries. They will help support the repeatprescription system, deal with acute prescription requests, and medicinesreconciliation on transfer of care and systems for safer prescribing, providingexpertise in clinical medicines advice while addressing both public and socialcare needs of patient in the GP practice(s).

Person Specification


Experience

* Previous experience that demonstrates good attention to detail.
* Experience of working in a confidential environment.
* Previous experience of working in General Practice, NHS or Healthcare setting.


Skills & Knowledge

* In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
* Good general computer skills, including the ability to use Microsoft Office, email and the internet.
* Evidence of excellent administration and organisational skills.
* An excellent standard of grammar and spelling in the English language.

* Able to obtain and analyse complex technical information.

Qualifications

* Mandatory registration with General Pharmaceutical Council
* Masters degree in Pharmacy (MPharm) or equivalent
* Membership of the Royal Pharmaceutical Society
