About the role:
1. Improve the outcomes for people exiting structured treatment by supporting service users to access the appropriate intervention, treatment and support at the earliest opportunity.
2. Support the service user to connect with activities, support to sustain recovery and reach their goals set out in their aftercare plan. training, employment support.
3. Undertake clear, recovery focussed, trauma informed support and management of risk.
4. Provide service users with information on options available them within the service and in their communities.
5. Maintain up to date, timely records on CRIiS (Data Management System).
6. Provide service users with advice and information to keep themselves safe.
7. Reduce barriers to engagement and be flexible in your approach to the accessing support. This may involve addressing language barriers, conducting outreach or virtual sessions.
About you:
Experience of conducting and developing personalized Service User / Aftercare plans.
8. Experience of delivering or supporting to deliver interventions.
9. Experience of inputting onto case management systems.
10. Knowledge of Warwickshire
