An exciting job role has recently become available at Sefton. They are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be?
As a Social Worker in the Adult Community Team’s Hospital Discharge Team in Sefton, you will assess patients’ needs for safe hospital discharge. You will develop and implement care plans in collaboration with healthcare professionals, patients, and their families. Coordinating community services to support patients’ transition home is a key responsibility. You will advocate for patients’ rights and preferences, ensuring they are respected and upheld. Regularly reviewing and adjusting care plans to meet patients’ evolving needs is essential. Maintaining accurate records of assessments and interventions is crucial. Additionally, you will collaborate with multidisciplinary teams to provide holistic care. Key skills for this role include strong communication, empathy, and the ability to work under pressure.

Qualifications and Experiences:
To be eligible for this role, you must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
