Role Description
This is a full-time on-site role for a Care Advocate based in Sunderland. The Care Advocate will be responsible for providing support and advocacy services to patients, ensuring their needs are met and their voices are heard. This includes assisting patients with accessing healthcare services, providing information on available resources, and coordinating with healthcare providers to ensure continuity of care. Additional responsibilities include maintaining patient records, participating in case reviews, and identifying opportunities for improving patient care.

* Strong interpersonal and communication skills
* Experience in patient advocacy and healthcare coordination
* Ability to work collaboratively with healthcare providers and staff
* Knowledge of healthcare systems and available resources
* Excellent organizational and record-keeping skills
* Empathy, patience, and a commitment to patient-centered care
* Ability to work on-site in Sunderland
