We are seeking a motivated and experiencedParamedic to join our multidisciplinary team. The successful candidate willwork across five practices, conducting home visits and supporting clinicalteams with a wide range of paramedic duties. You will play a vital role inimproving access to care, reducing pressure on GPs, and enhancing patientoutcomes.
Main duties of the job
Home Visits: Conduct clinicalassessments and treatments in patients homes, particularly for housebound andvulnerable individuals.
Minor Illness Clinics: Diagnoseand manage minor illnesses and injuries independently or with GP support.
Triage and Assessment: Supporttelephone and face-to-face triage, assessing patients and directing them toappropriate care pathways.
Chronic Disease Management:Assist in monitoring and managing long-term conditions such as asthma, COPD,diabetes, and hypertension.
Emergency Response: Provideurgent care and support in emergency situations within the practice setting.
Medication Reviews: Conductreviews and support prescribing within scope of practice.
Health Promotion: Educatepatients on lifestyle, prevention, and self-care strategies.
Documentation: Maintainaccurate and timely clinical records using SystmOne or other clinical systems.
Collaboration: Work closelywith GPs, nurses, pharmacists, and other allied health professionals to ensurecoordinated care.
Safeguarding: Recognise and acton safeguarding concerns in line with local and national policies.

Person Specification
Qualifications
* HCPC registered Paramedic
* Experience in primary care or community settings
* Strong clinical assessment and decision-making skills
* Excellent communication and interpersonal abilities
* Ability to work independently and across multiple sites
* Independent Prescriber qualification (or working towards)
* Experience with SystmOne or similar clinical systems
* Additional qualifications in chronic disease management, minor illness, or advanced clinical practice
