Job overview As a Mental Health and Wellbeing Practitioner (Band 5) within the Psychological Therapy Service at Inclusion Thurrock, you will play a pivotal role in supporting adults with severe mental health problems in a primary care setting. This is a permanent, full-time position (1.0 WTE) to ensure the team continues to meet essential KPIs, specifically the 28-day initial assessment target aligned with MHSDS standards. Working under the supervision of Senior Cognitive Behavioural Therapists or Practitioner Psychologists, you will provide evidence-based, psychologically informed interventions based on CBT principles. You will be joining a team navigating a significant period of restructuring and funding changes, making this role "an essential cog" in maintaining service flow, clinical performance, and staff morale. The post holder must hold a Level 6 or Level 7 graduate/postgraduate certificate in the national curriculum for Mental Health and Wellbeing Practitioners. Advert The primary focus of this role is the delivery of high-quality, collaborative care and evidence-based interventions for service users and their families. Key duties include: Assessment & Risk Management: Carrying out initial clinical assessments and risk management plans within the required 28-day timeframe. Clinical Interventions: Delivering specified 1:1 and group interventions, such as behavioural activation, graded exposure, problem-solving skills, and sleep hygiene. Care Coordination: Acting as a care coordinator to monitor progress, set collaborative goals, and support shared decision-making within a multi-disciplinary team. Clinical Supervision: Engaging in regular case management and clinical skill supervision to ensure the delivery of safe, high-standard care Relationship Management: Building empathic, professional relationships with service users in distress and liaising with external partners like GPs and voluntary agencies Working for our organisation Midlands Partnership University NHS Foundation Trust Inclusion service offers psychological and drug & alcohol services, in the community, and in prisons, and has contracts across the country. Clinical Practice: You will deliver psychologically informed interventions for issues like emotional dysregulation, confidence building, and binge-eating, while also providing medication support. Communication & Documentation: Maintaining accurate, up-to-date clinical records in line with Trust standards and providing highly effective written and oral reports for multi-disciplinary reviews. Professionalism & Safety: Upholding patient dignity, respecting cultural diversity, and adhering to infection control and safeguarding policies. You must be fit to undertake physical interventions for managing violence and aggression if required. Service Development: Participating in clinical governance, audits, and quality improvement programs, as well as supporting the induction and training of students and volunteers. Relapse & Discharge: Collaborating with users to develop relapse prevention plans and managing the ending of therapeutic relationships safely and appropriately.
