Overview
Weston-super-Mare, UK Remote Contract, Location: Home-based, North Somerset. Frequent travel will be required (to include team meetings or other work-related meetings). This is an exciting opportunity to work with stroke survivors and their families to support them following a stroke.
We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Stroke Recovery Service based in North Somerset. You will have experience in:
Responsibilities
* Provide person-centred support to empower vulnerable people or people with a disability or long-term health condition and their carers.
* Handle a variety of calls, potentially dealing with complex and challenging situations whilst working in your own home, nurturing emotional resilience.
* Facilitate face-to-face group work with vulnerable people or people with a disability or long-term health condition.
* Communicate clearly and effectively with a diverse audience including those experiencing communication difficulties via face-to-face visits or meetings, telephone calls, emails or letters, and digital methods (such as video calls).
* Travel extensively across the geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement.
* Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke.
Requirements / Qualifications
* Experience of providing person centred support to empower vulnerable people or people with a disability or long-term health condition and their carers.
* Experience in facilitating face to face group work with vulnerable people or people with a disability or long-term health condition.
* Effective listening skills with the ability to communicate clearly and effectively with a diverse audience including anyone experiencing communication difficulties.
* Ability to work remotely from home and manage a role that involves travel.
* To fulfil the role, you must be a resident of the UK and have the right to work in the UK.

* Salary: Circa £27,400 per annum
* Hours: Full-time, 35 hours per week
* Contract: Fixed-term until October 2026
