Overview
This post consists of a funded 2 year apprenticeship via HEE England completing MSc in Advanced Practice, commencing at Band 7. On completion of the degree, progression to Band 8A will be awarded on a permanent basis.
Role
The ACP's role is to assess, diagnose and treat patients independently referred from North West Ambulance Service, local GPs, care homes, wider community services, Virtual Ward and step down patients from the acute site. You will work alongside your ACP and GP colleagues to gain experience and a wider understanding of the ACP role in a Community Urgent Care Response service providing urgent assessment and wrap around care to avoid hospital admission or facilitate admission to CAU, SDEC or virtual ward. Other pathways in and out of CRT are being developed and this is an exciting time for the team.
Qualifications and Requirements
* Standard requirements for a post at this level (Nursing or AHP background).
* Minimum of 4 years' post-registration experience.
* At least a Grade C/4 (or equivalent) in Maths & English at GCSE level.
* Please note that these additional requirements have been set by the University of Bolton. If you do not list these on your application, you cannot be considered for the post as you will not meet the University's criteria for the training course.
Profile
We\'re looking for an ambitious, enthusiastic, motivated and experienced clinician who will be required to work as an autonomous practitioner identifying, assessing, diagnosing and treating adults with acute medical, frailty or complex needs to enable them to remain in place of residence and avoid hospital admission.

Hours
This is a full time post (37.5 hours) working across Monday to Sunday between 0800-2000. This post cannot be offered on a part-time basis due to the taught element.
